CPDC's programs for human services follow a new model for care, combining three elements to insure that people are connected to the resources they need:

Case management and service coordination which identifies residents' needs and links them to on- and off-site service providers; |

Collaborative service delivery, which integrates all efforts in a consistent and comprehensive approach. |
CPDC Human Services provides certain programs directly, primarily in the Edgewood Terrace community in North East Washington D.C.. Where CPDC's Human Services does not fill a need, it partners or coordinates with other organizations to provide services.
