Relocation Case Manager (Part-Time)

Position Summary

The primary function of the Relocation Case Manager is to coordinate services and the assist residents of Hollins House, a west Baltimore mixed population community, before, during, and after the renovation of their building. Hollins House is a 130-bedroom development with a mix of older adults and disabled non-elderly adults. The property is scheduled property to be under renovations until early 2017 and residents will be temporarily moved from their units for approximately two weeks. The Relocation Case Manager will work with each resident to make sure that his or her needs are being met and that the transition is smooth process. This position will report to the Neighborhood 4 Senior Manager, Community Impact Strategies and work closely with a Relocation Specialist. After the renovations are complete, this position will focus on case management and programming.

Essential Job Functions

  • Develop positive, professional, working relationships with the property’s residents and the property management staff
  • Help residents prepare for their temporary relocation during renovation
  • Coordinate any additional services that residents may need during their transition
  • Provide advocacy-based case-management to clients
  • Assess for need and provide move-in assistance for residents
  • Other duties as assigned

  • A willingness to occasionally work inconsistent hours, including weekends and evenings when necessary

Minimum Qualifications

  • Bachelor Degree in Human Services, Social Work or a related field
  • Case management experience, especially with older adults and/or disabled adults
  • Experience with relocating older adults and/or disabled older adults strongly preferred
  • Ability to work under limited supervision and considerable self-direction
  • Team player who is resourceful, proactive, and comfortable working with diverse populations
  • Strong communications skills: listening, oral and written and positive, problem-solving
  • Demonstrated ability to perform job duties within the context of complex regulations and definitions and to complete extensive technical documentation.
  • A working knowledge of the local social service system or the ability to quickly develop such knowledge
  • Ability to identify, assess, select, develop and maintain community service referral partnerships that assist residents in achieving their goals
  • Excellent interpersonal, verbal, and written communication skills
  • Competent computer and technology skills
